Submission Deadlines and Non-Compliance Consequences

All students are required to comply with their school and program-specific deadlines. Failure to do so will result in a block on course registration. Students enrolled in dual-degree programs should select their primary school/division affiliation.

A $100 fee will be charged to have the health form completed at Student Health & Well-Being Primary Care.

International students and learners who are unable to receive the TDAP vaccine in their home countries before coming on campus will have the administrative fine waived if that is their only missing requirement.

Note: School of Nursing students continue to follow their school’s policy. Please refer to your school’s requirements and submission guidelines.

Non-Compliance Consequences

Students who remain non-compliant after the compliance deadlines listed below will have a hold on their course registration in the Student Information System (SIS). A registration hold means you cannot add, change the grade system, or register for future classes if you have not met the immunization requirements.
