Yawns Hopkins Sleep Kit Request Form (Homewood)

Requests will be fulfilled within two business days. Pickups will be available from 9:00am to 5:00pm at our Homewood office at the Ralph S. O’Connor Center for Recreation and Well-Being Level 2, Recreation and Well-Being Suite 3400 N. Charles St Baltimore, MD 21218. We will hold your kit for you for two weeks from the day you request it.
